If two editors have got it confused, you can be sure two readers will have it confused too.

The best general rule for all these sorts of things is CLARITY.

It's not the rule/convention so much as the ease of understanding that matters.

Just get Final Draft.

A lot of it is self-explanatory or easily understood. FD does all the work for you, leaving you to focus on story.

Believe me, you'll save yourself a lot of hassle.

It's just shorthand for effective communication.

You can modify to suit purpose. For example, INT. NIGHT 2AM vs INT. NIGHT 4AM

The important thing is that the message gets across succinctly.

Linear timelines help to keep things clear. You can tell it non-linearly, just don't confuse the audience.

You can detail the character in a second appearance. You don't even need to do that, you can just reveal little bits as needbe as you go along.
